The facts set forth in this application and any supplemental information are true and complete to the best of my knowledge. I understand that, if employed, falsified statements on this application shall be considered sufficient cause for immediate discharge. I hereby authorize investigation of all statements contained herein and employers listed above to give you any and all information concerning my employment, and any pertinent information they may have, and release all parties from all liability for any damage that may result from furnishing same. I understand that neither the completion of this application nor any other part of my consideration for employment establishes any obligation for the company to hire me. If I am hired, I understand that either the company or I can terminate my employment at any time and for any reason, with or without cause and without prior notice. I understand that no representative of the company has the authority to make any assurance to the contrary. I understand that I am required to abide by all rules and regulations of the company.

I understand that Central Mass Transit Management Inc. and Paratransit Brokerage Services TM Inc. abides by an employment-at-will policy except where there is a collective bargaining agreement in effect. In such circumstances, the terms of the collective bargaining agreement contract take precedence.

However, for employees not covered by a collective bargaining agreement, the following terms apply: Either the company or the employee may terminate the employment relationship at any time, for any reason, with or without notice. Additionally, nothing contained in this employment application or anything conveyed during an interview, is intended to create an employment contract, implicit or implied. I also understand and agree that any future changes in my titles, duties, compensation, working conditions, benefits or company policies and procedures will not alter this at-will agreement. I understand that Central Mass Transit Management Inc and Paratransit Brokerage Services TM Inc. maintains a Drug and Alcohol-free workplace and requires a drug-screening test as a requirement for employment. I understand that the information presented here is only intended as a brief summary of the Central Mass Transit Management Inc. Drug and Alcohol policy and that the complete policy is available for review from the Human Resources department by request.

I understand any offer of employment will be conditional upon my successful completion of a physical examination and a pre-employment drug test.

I understand that failure to take or pass a physical examination and drug test at any time during my employment may be sufficient cause for dismissal from the company.

I further agree to submit to physical examination, including a substance abuse test when requested by management.

I understand that continued substance abuse testing during the course of my employment is a condition of continued employment.

I further agree to abide by the existing rules of the company and by any rules and regulations as may become effective during my employment.

I hereby certify that all statements made in this application are true and correct to the best of my knowledge.

I authorize Central Mass Transit Management or Paratransit Brokerage Services TM Inc. to investigate all statements contained in the application.

I understand and agree that any false statements, misrepresentations or omissions of facts appearing on this application or any other employment form or medical information/examination form will result in an immediate dismissal or removal of my application from consideration or for immediate discharge if I am employed, regardless of the time elapsed before discovery.

I authorize CMTM OR PBSTM to secure information about my experience with former employers, education institutions and agencies, and for those parties to provide information concerning my experience releasing all parties from any liability arising there from.
